PATIENT AND VISITORS GUIDE

Pre-Admission

  1. Prepare your identification cards indicating personal information, such as your current address, phone number, next of kin, and employer. It is best to bring a valid government-issued photo ID. Patient information must be complete and accurate.
  2. Plan for your payment arrangements. Bring your insurance documents or medical coverage ID cards, In case of emergency HMO card and valid ID are not available, admitting staff will report the case to health care provider and the patient will be given first all diagnostic procedures, emergency drugs or any emergency services immediately.
  3. All HMO Cardholder must be declared within 24 hours upon admission or else the hospital will consider the confinement as cash out.

Financial Agreement/Guarantee of Payment

  • For emergency cases, after 24 hours, a partial payment is requested by business office staff.
  • For elective cases, a partial payment depends on patient case is required.

The In-House Collection Officer will request for additional payment of seventy five percent (75%) during confinement and full payment upon discharge. The Business Office provides you with a statement of account daily. Once partial payment has been consumed, you are advised to update your account to cover all additional services provided to you.

  • All HMO members are also required to pay whatever excess and charges not covered by HMO.
  • Coverage will be based on final diagnosis of accredited physician. If in case the patient will undergo operation the coverage will be based on RVS code given by accredited physician.

If the patient decides not to proceed with the inpatient admission, ER Doctor of Laguna Holy Family Hospital will assist the patient in his or her transfer to another hospital of the patient’s choice.

Important Documents

Our goal is to help you get settled and feel comfortable in the hospital environment as quickly as possible. We encourage you to have with you the following documents (as applicable):

  1.    Personal Identification (ID) with photo
  • Accredited HMO-issued member’s ID
  • Senior Citizen’s ID
  • PhilHealth ID
  • Persons with Disability ID
  • Healthcare insurance card or document
  • PRC ID
  • Unified Multi-Purpose ID
  • Passport

The  hospital  admissions  personnel  will issue  you  a  unique patient identification number in a bracelet  indicating your  full  name,  date  of  birth,  a  patient  ID number, and the name of your Attending Physician. The identification (ID) bracelet must be worn at all times during your hospital confinement. DO NOT REMOVE the ID bracelet until you are discharged from Laguna Holy Family Hospital. This is for your own safety.

For Room Allocation and Room Reservation

  • The Admitting Staff are responsible for room allocation and room reservation. Priorities in room/bed queuing will be given to patients with emergency cases.
  • In cases, where no preferred rooms are available, patients/relatives will be offered alternative rooms and will be transferred to the room of choice upon availability.
  • The Attending Physician requests for a room reservation to Admitting Staff.
  • Room reservation is subject to the availability of rooms. The assigned room shall be held for patient up to a maximum of four (4) hours beyond the patients scheduled time of arrival. If the patient fails to arrive within the maximum allowance, the queued room/bed will be assigned to the other waiting patients, if any.
  • Laguna Holy Family, Inc. has right to cancel room reservation to give way to emergency and critical cases.

Philhealth

  • If Philhealth member, he/she will be provided a list of requirements and forms required to avail Philhealth deduction from final bill. A Philhealth Officer will explain the requirements in detail.
  • Philhealth member or relative is instructed to process and completely fill up required documents ahead of time to avoid delays during discharge.
